Full conference
Full conference registration includes Monday and Tuesday program sessions, exhibits, luncheons, breaks, Welcome Dessert Reception, and the Kentucky Derby Museum Reception.

One-day registration
One-day registration includes all program sessions, exhibits, luncheon and breaks on the specified day—Monday, October 9 or Tuesday, October 10.  Tickets to the Welcome Dessert Reception and the Kentucky Derby Museum Reception may be purchased separately.

Student registration
For Library/Information Science students.  Full conference registration for students includes Monday and Tuesday program sessions, exhibits, luncheons, breaks, Welcome Dessert Reception, and the Kentucky Derby Museum Reception.  One-day registration, which does not include social events, is also available.

Membership
Member rates are for current members of the Midwest Chapter.  If you’re uncertain about your membership status, contact Bette Sydelko, 937-775-3840 or bette.sydelko@wright.edu.

Continuing Education Courses
Continuing Education Courses are charged separately. Those attending CE events only do not need to pay an additional conference registration fee.

Space may be limited by request of instructor or because of classroom size for some CE classes—register early.  CE classes may be cancelled due to insufficient enrollment.  You will be notified if a class for which you registered is cancelled and a refund will be granted.

Additional Tickets for Special Events
One ticket is included in full conference registrations, but additional tickets may be purchased for guests, spouses, significant others, others wishing to attend, or by one-day attendees.

Completing Your Registration
REGISTRATION WILL BE BY MAIL ONLY (sorry-no online registration this year!) 

Print and complete the registration form.   Make checks or money orders payable to Midwest Chapter/Medical Library Association, or provide credit card details (Visa, MasterCard or Discover).  Credit card transactions will be posted after July 1, 2006.  Mail the form with your payment to:

Confirmations will be sent by email; receipts will be in your registration packet when you arrive.

On site registration will be available, subject to space or headcount limitations. If possible, please register in advance.

Registration/Hospitality Hours
The Registration/Hospitality Desk will be staffed in the Louisville Marriott Downtown during the following hours:

Saturday, October 7, 2pm-8pm
Sunday, October 8, 7:30am-8pm
Monday, October 9, 7:30am-8pm
Tuesday, October 10, 7:30am-8pm
Wednesday, October 11, 7:30am-1pm

Please stop at the desk for assistance with the schedule, directions to events, information about Louisville, or just to say hello.

Your registration packet will contain a badge and tickets to ticketed events.  Please keep the badge and tickets with you during the conference; you may be asked to show them at sessions, exhibits and other events

Cancellations
Cancellations must be received in writing before October 1, 2006.  Print the refund request form provided, fill it out, and mail it to Judy Wulff at the above address.  Refunds will be made by check only after October 23, 2006.  Payments made by credit card cannot be refunded back to the card, but will be refunded with a check.  All refunds will be subject to a $10 processing fee.
