Course Reserves Personnel
Policies: Art Library, Ekstrom Library, Kornhauser Health Sciences Library, Music Library

This form allows faculty (or their designees) to request that materials needed by a class be held at a check-out desk in one of the libraries. These materials can then be checked out for limited amounts of time. Please submit one form for each course. This form has space for up to 10 items; please submit additional items on a separate form.
